Reason for withdrawing (optional — you never have to give one) Your right of withdrawal does not depend on a reason, and leaving this empty has no effect whatsoever on your refund or how fast it arrives.

5 · Your declaration

This is exactly what will be recorded and repeated back to you in the acknowledgement.
